We asked our residents, community partners, and staff to describe a healthy community. Their responses mirrored the social determinants of health, without using that somewhat confusing phrase. We were able to use the communities’ words to develop a graphic depiction of a healthy community. This served as an organizing framework for our 2018 Community Health Assessment and a catalytic mechanism for working with communities.
