Successful public health interventions rely on good data, and having good data requires intentional and inclusive community-based collection efforts.  This session highlights data collection efforts from the Pueblo County Community Health Assessment and Communities that Care Program, specifically those aimed at stakeholders, youth and minority populations. Session attendees will be encouraged to discuss and apply discussion concepts to improve processes of both county and program level data collection for assessment, planning and evaluation purposes.
